My child has permission to attend and participate in the Herb Haygood Off Season Skills Camp.  I hereby give my approval to the participation in any and all of the activities of the camp 2007 included but not limited to practices, drills, games, if any, and any other activity associated with the camp on and after the date hereof.  I authorize the directors of the Herb Haygood Off Season Skills Camp to act for me according to their best judgment in any emergency requiring medical attention and I hereby waive and release the Herb Haygood Off Season Skills Camp from any and all liability for any injuries and illnesses incurred while at the Herb Haygood Off Season Skills Camp.  I will be responsible for any medical or other charges in connection with my son’s attendence.  I know of no mental or physical problems which may affect my child’s ability to safely participate in this program.  I understand that the Herb Haygood Off Season Skills Camp may take photographs of participants and activities.  I agree that the Herb Haygood Off Season Skills Camp shall be the owner of and may use such photographs relating to the promotion of future events. I relinquish all rights that I may claim in relation to use of phototgraphs.  I understand that violation of camp rules may result in dismissal from camp with all tuition forfeited. 

Cancellation/Refund Policy
If you cancel at least four weeks prior to the beginning of the camp for which your child is registered you will receive half of your registration fee back or credit towards a future Herb Haygood Off Season Skills Camp.  There will be no refund if you cancel less than four weeks prior to the start of camp or upon expulsion from the camp.
